Access Your Southeastern Medical Center Medical Record

How to request a copy of your medical records

To request your medical records from Southeastern Medical Center please download, fill out and submit our Release of Information Authorization.

There are four ways you can send us this information:

  1. Fill it out electronically and email it to us at SMC-MedicalRecords@OhioHealth.com
  2. Print, fill out & fax it to 1-740-439-8658
  3. Print, fill out & mail it to: 1341 N. Clark St. Cambridge, OH 43725 Attn: Medical Records
  4. Print, fill out & bring it in person to the Medical Records Deptartment

You will need to provide the following:

  • Name & DOB of patient requesting records
  • Visit location/date, as well as description of records needed
  • Patient signature/witness signature (other than family)
  • Legible copy of photo ID

How do I get a copy of my records?
Call (740) 439-8652 Monday - Friday 8 AM to 4 PM.

How do I get a copy of diagnostic images on CD?
Call our Radiology department at (740) 439-8036.

I would like to get a copy of a family member’s records. Can I do this?
Only the patient may release his/her records. However, if you have the power of attorney or are the executor of a patient’s estate, you may submit a request in writing along with a copy of the legal document.

Do you release any information verbally?
The only information Medical Records releases verbally is date of admission and date of discharge.

Can you help me find out my blood type?
We do not give patients their blood type, for the only time there would be record of this is if the patient needed to be crossmatched for transfusion purposes or had a surgery at our facility. Please contact your local health department:

I was born at Southeastern Ohio Regional Medical Center. How can I obtain my birth certificate?
